What is MyCHOP?
MyCHOP provides patients, parents, and legal guardians’ online access to portions of the electronic medical record (EMR). The available information includes lab results, appointment information, medications, and immunizations, all securely stored for quick retrieval. MyCHOP also provides a convenient method of communication with your doctor’s office to renew prescriptions, send messages, and schedule appointments.

MyCHOP is available to patients 13 years and older, and to parents and legal guardians of patients 8-18 years old who have been treated by a CHOP physician.
Is MyCHOP secure?
Yes, CHOP takes great care to ensure all health information is kept private and secure. Access to information is controlled through secure access codes, usernames, and passwords. Each person controls his or her password, and the account cannot be accessed without that password. MyCHOP also uses the latest 128-bit SSL encryption technology and unlike conventional email all MyCHOP messaging is done while you are securely logged on to our website.

CHOP has in place detailed standards regarding access to all medical records by our staff and employees and has carefully outlined the circumstances under which your medical information may be released to parties outside the organization. These standards conform to state and federal law and are designed to safeguard your privacy. Your name and email address will be treated with the same care and privacy given your health records and will never be sold or leased by MyCHOP.

If you do not remember that email address, you will have to contact your healthcare provider to help you regain access to your MyCHOP account or request MyCHOP help.
Technical support is available M-F 8am-4pm. Messages received after this time will be addressed within 5-7 business days.
How can I get a complete set of my child's medical records?
You can obtain your child's medical records by contacting your child's CHOP healthcare provider's office.
Return to TopWhen is proxy access revoked?
At age 18 the proxy access will be revoked. Patients have the option of having their own MyCHOP account and/or they can sign the "Patient Approved Enhanced Access" form which grants an individual access to the adult patient's medical record.
Return to TopWill my physician contact me through MyCHOP?
Yes, when your physician releases new test results they may send you a note attached to your results or a secure patient message to help you understand the significance of the test result. They may also recommend that you make a follow-up appointment, provide you with additional instructions, or ask you to contact the office to discuss your test results. A notification may be sent to the email address on file within MyCHOP to alert you when a new test result or message is available in MyCHOP.

Yes. You can use MyCHOP messaging to send messages and requests to the office of your physician. Currently, the following messages can be sent via MyCHOP.

How do I communicate urgent medical issues?
MyCHOP is not used to communicate urgent medical issues.
For urgent medical matters, please call 911 or your local emergency services provider.

Due to the complexity of scheduling specialty appointments, you cannot request an appointment for all specialties via MYCHOP. Please call your specialist's office to schedule an appointment if the option is not available in MyCHOP.
Return to TopWhy don't all of the medications show up in MyCHOP?
Prescriptions written by non-CHOP physicians will not appear on your medication list unless they are documented by your CHOP physician in your CHOP Electronic Medical Record. Please contact your CHOP Health Care providers for any questions regarding your Medication list.

You should provide the following information for a prescription renewal:
- Your preferred pharmacy.
- Any comments that you wish to include.
Who sees my request for prescription refill?
Prescription refill requests are sent via secure messaging to the office of the physician or practitioner that ordered the medication.

Every attempt is made to process prescription renewals within 2 business days of receipt during normal business hours. If you submit your request on a weekend or holiday, it will be received and processed within 2 business days of the next business day.

The “Request a Renewal” link will not be available for medications that were prescribed by a CHOP physician who is not currently using MyCHOP.

Click on the "My Medical Record" menu and select the "Test Results" link. On the "Test Results" page, click anywhere within the row to display the results for that test.
Return to TopWhy do some of my child's test results have various descriptions (Final, Result, Edited)?
A Final Result is a Result that is complete and filed. An Edited result is a Finalized or "Final" result that your CHOP health care provider or nurse added comments or changes to that result.
Return to TopMy child had a test last week but I don't see the results. Why?
It may take several days or even weeks for certain tests to be resulted. If you feel that your test results should be available but you don't see them in MyCHOP, it is best to contact your CHOP healthcare providers office to discuss the test results.
Return to TopWhy don't I see the results of all the tests that my CHOP healthcare provider ordered?
Results will only appear in your MyCHOP account for tests that were ordered by CHOP healthcare providers from within departments that support MyCHOP. If a test that was ordered from within a department that does not support MyCHOP, the Result will not appear in MyCHOP. If you have any questions regarding any of your results, you may contact your CHOP healthcare provider's office.

Radiology Results finalized after 3/29/2016 will be auto released to MyCHOP. Radiology results finalized before 3/29/2016 will need to be manually released by your doctors office. Please contact your office for those results. Images are NOT available in MyCHOP.

What is an access code?
Your access code is a unique identifier that will allow you to login to MyCHOP for the first time. You will also be required to enter your date of birth and zip code at this time. The access code will enable you to log into MyCHOP and create your own user ID and password.

There are several reasons why your access code may not be working.
- Access codes expire 90 days after the date they were issued if you have not registered for MyCHOP.
- You are required to enter your Date of Birth and Zip Code when registering for MyCHOP. Verify that you are entering this information correctly.

No, your access code is not your MyCHOP username or password. You will use the access code that you are given from your CHOP healthcare provider when logging into MyCHOP for the first time. You will be prompted to enter a username and password during the initial login. Access codes expire 90 days after they were assigned. If your access code has expired, please contact your CHOP Healthcare provider or request MyCHOP help.
Return to TopHow do I choose my personal MyCHOP username and Password?
The first time you login to MyCHOP, you will be prompted to create a username and password. This is the username you will use during subsequent logins to MyCHOP. Once selected, your MyCHOP username cannot be changed. Choose an ID and password that is unique to you and easy for you to remember. Avoid using an ID or password that is easy for others to guess, such as your first or last name. Your ID must be between 6 and 18 characters long. Only letters and numbers are allowed and it cannot contain any spaces. It must contain at least one number and one letter. Also, remember to enter an email address so that MyCHOP can notify you when new information has been added to your chart.
Return to TopHow does the password reset question and answer work?
The password reset question and answer set will allow you to reset your password if you forget it or allow you to change your password at any time you wish. When you login the first time and setup your personal MyCHOP user ID and password, you can also specify a password reset question and matching answer. Common password reset questions are: “Where were you born?” and “What is the name of your first pet?” Once you select your password reset question, you will be prompted to enter an answer to the question. In the future, if you have forgotten your password or simply wish to change it, you can click on the Forgot Password? link on the login page. The system will ask you the password reset question. If you correctly answer this question, you will be allowed to reset your password. (Please keep in mind that your password reset answer cannot be the same as your MyCHOP password.)

Return to TopWhat if I forget my MyCHOP username?
Your MyCHOP username is permanent and cannot be changed. If you forget your MyCHOP username, you have 2 options to proceed with:
- You can also recover your Username by accessing Forgot your MyCHOP username?
- Please call your CHOP healthcare provider's office or request MyCHOP help. You will be given a new access code to sign in to MyCHOP. Once you have entered your new access code along with your date of birth, your original MyCHOP username will appear. You will then be asked to select a new password.
Can I change my password?
Yes. Your password can be changed online at any time by choosing “Change Password” option under the Preferences link. Please keep your password confidential and do not share it with anyone. If you believe that someone has discovered your password, please change it.

Ask your doctor to correct any inaccurate information. Contact your CHOP healthcare providers' office, or ask your CHOP Health Care Provider in person. Your health information is reviewed and can be updated in your electronic medical record during each visit.

Your demographic information for each child can be found by logging into MyCHOP, selecting your child, then on Profile menu at the top of the screen, select Personal Information.
Make your corrections and click “Accept Changes”. A message will be sent to your physician's office with your new information and MyCHOP will be updated.
How can I update my address in MyCHOP?
From the Health menu at the top of the screen, select Change Demographics. Make your corrections and click “Accept Changes”. A message will be sent to your physician's office with your new information and MyCHOP will be updated.

Updating your email address for your MyCHOP account: From the Personalize screen after login, select Profile, then Personal Information. Enter your new email address and then click the “Submit Changes” button. This is the email address on file for MyCHOP notifications.
To update your email address account on your child's demographic info (your child's chart): Login to MyCHOP, select your child, then from the Profile menu at the top of the screen, select Personal Information. Make your corrections and click “Accept Changes”. A message will be sent to your physician's office with your new information and MyCHOP will be updated.
Why do I need an email address to use MyCHOP?
When new information is sent out to your MyCHOP account, you will also get an internet email to let you know that new information is available.

Children’s Hospital of Philadelphia has the ability for patients to be seen by their providers without having to come in for an in-person visit. CHOP providers use video to see patients and talk about their health needs.
Return to TopWhat do I need for a MyCHOP Video Visit?
Patients and families would use their mobile phone, tablet, or a computer with a webcam to join a video visit with a CHOP provider. It is strongly recommended that you use a mobile device (phone or tablet) and to use Wi-Fi when possible, instead of your cellular network.

These visits would be scheduled, just like an in-person appointment. Talk to your provider to find out if it makes sense clinically to have a video visit instead of an in-person visit.

Access the portal via a web browser at MyCHOP
After logging in, open the letter you wish to print. Select the printer icon located on the upper-right corner of the window. A preview screen will display. Click the PRINT THIS PAGE hyperlink displayed at the top of the page. Select Microsoft Print to PDF option from the print window. Select the directory and enter file name to save document. Click the Save button to save the letter as a PDF document.

If using the mobile app: Navigate to the messaging feature by tapping the message icon > tap send a message > select a recipient > select a subject > tap attachments. Here you can take a photo with your camera or chose a photo > enter a message > tap Send.
If using the web portal: Navigate to the messaging feature by hoovering on the MSG icon and click Contact Your Healthcare Provider > Select a Recipient > Select a subject > Enter a Message > Click the Attach Image Button > Click Send.

Yes, multiple proxies (parents/caregivers) can join the video visit, as long as they have their own MyCHOP account. Parents/caregivers can join a video visit from different devices/locations. There is a limit of 5 participants in a video visit. That includes the CHOP Care team, parents/caregivers and patients.
